In keeping with CDC guidelines, all Westminster Baptist 3D Kids leaders and student helpers are no longer required to wear masks while in our facility.  However, 3D Kids workers may choose to wear masks if they desire.  In addition, all children attending 3D Kids activities are not required to wear masks or social distance inside our building. Children who are sick or have a fever must remain home until they are well or have had a COVID test, whichever is relevant.  To promote a healthy environment, hand sanitation stations are located throughout our building.

In the event of an emergency and the parent cannot be reached, I, the undersigned parent or legal guardian, do hereby authorize adult volunteers of Westminster Baptist Church as agent(s) for the undersigned, to consent to any medical or surgical care advisable by any accredited physician or surgeon in an approved emergency clinic or hospital.  I likewise authorize volunteer personnel to administer basic medical care such as applying band aids when deemed medically necessary.  I affirm that all medical and emergency contact information listed at registration is accurate to the best of my knowledge.  I further release from any liability Westminster Baptist Church and any of its ministries, leaders, or volunteers in the event of an accident before, during, or after any 3D Kids activity.  This agreement does not apply to claims for intentional misconduct or gross negligence.
