In the event of an emergency, and the parent cannot be reached, I, the undersigned parent or legal guardian, do hereby authorize adult volunteers of Westminster Baptist Church as agent(s) for the undersigned, to consent to any medical or surgical care advisable by any accredited physician or surgeon in an approved emergency clinic or hospital.  I likewise authorize VBS trained nurses to administer basic medical care such as applying band aids when deemed medically necessary. I affirm that all medical and emergency contact information listed at registration are accurate to the best of my knowledge.  I further release from any liability Westminster Baptist Church and any of its ministries, leaders or volunteers in the event of an accident before, during, or after Vacation Bible School. This agreement does not apply to claims for intentional misconduct or gross negligence.
